1. INTRODUCTION |
| This manual explains the purpose
and content of a program titled "THE RIGHT CHOICE FOR A
DRUG FREE WORKPLACETM ("Program").
This manual also outlines the procedures for implementing the
Program, provides instructions to management for training
employees subject to testing, covers detailed information about
controlled substances and alcohol, and presents a standardized
Program for all employees subject to testing under the Program,
and supervisory and management employees who are, in addition,
responsible for administering the Program.

| Substance Abuse / Drug Free
Workplace Policy
This section contains the Company's policy on pre-employment and employee testing for drugs as part of the Company's commitment to maintain a drug-free work environment. The policy is designed to inform employees of their responsibility in this process.

| Program Requirements and
Procedures
This section explains the purpose of the Program, identifies the categories of employees who will be subject to testing under the Program and gives detailed procedures for each of the four types of testing. Also included is information as to the responsibilities for those who administer the Program

| Supervisory and Management
Training
Management and supervisory employees who are responsible for determining whether reasonable suspicion exists, requiring an employee to undergo testing, must receive training on indicators of probable use of controlled substances. This training course consists of written instructional materials on alcohol and controlled substances, Reasonable Suspicion Testing Procedures and the use of the Record of Observed Behavior for Reasonable Suspicion Testing form Management and supervisory employees who are responsible for administering Pre-Employment Drug Testing, Random and Post-Accident Drug Testing should receive training on the procedures to be followed during the testing process and the forms to be used.

| Employee Training
This section contains basic information regarding the Program, its purpose and explains the types of testing included in the Program. The section also explains the consequences for violating the Program. The trainer photocopies certain pages from the Forms section and provides copies to all employees covered by the Program. All employees subject to testing under the Program must complete a basic training course and receive the Controlled Substances and Alcohol Use Handout to introduce them to the Program and its purpose. The handout includes an overview of the signs and symptoms of drug abuse and alcohol misuse. The Employee Training section also provides step-by-step instructions to guide the training leader who conducts the training session.

| Forms
The Forms section contains various forms that the training leader photocopies and distributes to each employee attending the training session. These forms are used for the following purposes: Obtain applicant and employee consent to testing. Notify applicants and employees of testing requirements and procedures (note: there is a separate notification form for each type of testing), record observed behavior for reasonable suspicion testing, and obtain acknowledgement by employee of having completed training.

This section contains a Glossary of Terms that defines the most common terms used in the Program. Also included in this section is a form that the Company can use to list the specimen collection site(s) and address(es). |
